First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any x-rays if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring your insurance card with you to your appointment. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)
X-Rays
If your general dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office, however, we typically will still have to take new images.
You may register with our office by downloading and completing our Patient Endodontic Packet. Please print and bring these forms with you on your initial visit.
FREQUENTLY ASKED QUESTIONS
We are only in network with Guardian Dental Insurance. We file with many plans out-of-network. After verifying your benefits, we will provide you with an estimated out-of-pocket. Even if we do not file with your insurance, we will fill out the claim form for you as a courtesy so please provide your information at the time of scheduling your appointment. If we do not receive that information from you promptly, we cannot complete the claim form for you. We do not file with Medicare as they do not cover root canals. In these cases, we will need to collect in full on the date of services rendered. In addition, endodontic care is not covered under any medical insurance.
Payment is due in full at time of service. We accept all major credit cards and Care Credit. We do not accept checks over $250.
Our office hours are 7:45am – 5:00pm Monday – Thursday. If you need to reach us after hours regarding an urgent matter, please call and you will be in touch with our answering service. Our texts are monitored during office hours only.
Crowns are usually required following endodontic treatment. We do not place crowns here; you will need to return to your general dentist for permanent restoration following your root canal treatment. If your tooth requires extraction, we will refer you back to your general dentist or to an oral surgeon.
